Become a PhilHealth Member

  • How do I become a PhilHealth Member?

    Go to any Ventaja’s agent and accomplish the PhilHealth Member Registration Form (PMRF). Expect to receive the PhilHealth Identification Number (PIN) after 2 working days.

  • Who can be my dependents

    The qualified dependents are the following:

    • - Legitimate spouse who is not a member
    • - Children below 21 years old
    • - Children above 21 but suffering from physical or mental disability.
    • - Parents who are 60 years old or above.
    • - Parents with permanent disability regardless of age.
  • How do I enroll my dependents?

    Submit the PMRF with the copies of the following documents:

    • - Legal spouse - Marriage Contract / Certificate
    • - Stepchild/children - Marriage Contract / Certificate between the natural parent and the stepfather/mother and Birth Certificate/s of the dependent stepchild/children
    • - Disabled child/children 21 years old and above - Birth Certificate and Certification from attending physician stating dependent is disabled
    • - Adopted child/children - Court Decree of Adoption
    • - Parents - Birth Certificate of member and any proof attesting to the date of birth of the parents
  • Who is eligible to become a PhilHealth member?

    All Filipinos regardless of age.

  • I already paid for PhilHealth before. Do I need to apply again?

    If you were already a member and would like to renew, simply provide your PIN to Ventaja’s agent and pay.

  • I do not remember my PIN. What do I do?

    Email the member’s complete name and birthdate to [email protected].

  • How do I get my PhilHealth Card?

    You may pick up your card where you applied for membership. It may take 60 days to process.

  • How much is the premium contribution?

    P2400 per year.

  • When does my coverage expire?

    One year after payment.

  • Can I buy PhilHealth for my relatives in the Philippines?

    Yes. Ask your relatives to complete the documentation and submit these to Ventaja’s agent.

  • How do I get my Member Data Record (MDR)?

    If you paid your PhilHealth premium thru a Ventaja agent, you may email your request for MDR along with the member’s PIN, complete name and birthdate to [email protected]. PhilHealth processes MDRs by batch and may take up to one week.



Benefits

  • When can I start using my PhilHealth?

    OFWs may start using PhilHealth once they receive their MDR. However, coverage starts when PhilHealth records the payment.

  • How will my dependents in the Philippines avail of my PhilHealth?

    Just submit the accomplished PhilHealth Claim Form 1 and a photocopy of your updated MDR.

  • Is confinement abroad covered by PhilHealth?

    Yes. The OFW has 180 days or 6 months upon discharge to file their reimbursement with PhilHealth.

  • Will PhilHealth cover doctor’s consultations?

    Yes. Members and their dependents must register first at their chosen Philippine government hospital. Members may choose only one hospital for consultations.

  • What do I need to bring for doctor’s consultations?

    Members and their dependents just need to bring their MDR.

  • What are the cases that are not covered by PhilHealth?
    • - Fifth and subsequent normal obstetrical deliveries
    • - Non-prescription drugs & devices
    • - Alcohol abuse or dependency treatment
    • - Cosmetic Surgery
    • - Optometric Services
    • - Cost-ineffective procedures that shall be defined by PhilHealth

  • How do I get my reimbursement if confined in the Philippines?

    Hospitals should deduct the allotted case rate from the total bill. Many hospitals have PhilHealth reps present.

  • How do I get my reimbursement if confined abroad

    Submit the following to PhilHealth within 180 days:

    • - PhilHealth Claim Form 1 accomplished and signed by the member or his authorized representatives
    • - Photocopy of MDR or latest receipt
    • - Medical certificate with complete diagnosis, period of confinement and services rendered (in English)
    • - Statement of Account and/or Official Receipts with itemized charges (in English)
  • I am currently working abroad. Who can sign in my behalf on PhilHealth Claim Forms?

    - Any dependent 18 years old and above.
    - Authorized representative. Just show an authorization letter, as well as photocopies of 2 valid IDs of the authorized representative and the member.

  • How do I change the name appearing on my Member Data Record (MDR)?

    Just email the accomplished PMRF to [email protected] together with the scanned copy of any 2 of the following documents:

    • - Birth Certificate
    • - Marriage Contract/Certificate, (if married)
    • - Passport
    • - NBI Clearance
    • - PRC ID
    • - SSS/GSIS ID
    • - Postal ID
    • - Driver's License
    • - Alien Certificate of Registration or (ACR)
    • - Duly notarized Joint Affidavit of Two (2) Disinterested Persons attesting to the veracity of the name of the person requesting for amendment
  • How do I change my date of birth?

    Just email the accomplished PMRF to [email protected] together with the scanned copy of any 2 of the documents mentioned above except for PRC ID and joint affidavit.

  • How do I change the civil status appearing on my MDR?

    Just email the accomplished PMRF to [email protected] together with the scanned copy of the required document:

    • - From single to married - Marriage Certificate
    • - From married to widowed - Death Certificate of spouse
    • - For marriage annulled - Annulment Papers
  • How do I add dependents?

    Just email the accomplished PMRF to [email protected] together with the scanned copies of the required documents:

    • - Legal spouse - Marriage Contract / Certificate
    • - Legitimate or illegitimate children - Birth Certificate
    • - Stepchild/children - Marriage Contract / Certificate between the natural parent and the stepfather/mother and Birth Certificate/s of the dependent stepchild/children
    • Disabled child/children 21 years old and above - Birth Certificate and Certification from attending physician stating dependent is disabled
    • Adopted child/children - Court Decree of Adoption
